I wanted to share a clear, evidence-based overview regarding the risk of sexually transmitted infections (STIs) from unprotected oral sex.
There isn’t a single percentage that applies universally, as risk varies depending on the specific infection, presence of symptoms (such as sores), and individual factors. However, the overall picture is well understood.
In general, oral sex carries a lower risk than vaginal or anal intercourse, but it is not risk-free.(we all know this)
For HIV, the risk from oral sex is extremely low. The estimated per-act risk is close to zero, particularly when receiving oral sex (good for us receivers). The risk increases slightly if there are cuts, bleeding gums, or ejaculation in the mouth, but it remains very low overall.
For other STIs, the risk is more significant:
- Gonorrhea: This is one of the most commonly transmitted infections via oral sex. Estimated transmission risk can range roughly from 5–20% per exposure.
- Chlamydia: Lower transmission efficiency than gonorrhea, with an estimated risk around 1–5% per exposure.
- Syphilis: Transmission risk can be relatively high (around 10–30%) if a sore is present, even if it is not visible or painful.
- Herpes (HSV-1/HSV-2): Transmission is common, especially if the virus is active (e.g., cold sores).
- HPV: Also transmissible through oral-genital contact, with risk accumulating over time rather than per single exposure.
From a Punter - SW perspective:
- Receiving oral sex carries negligible HIV risk but a moderate risk for infections like gonorrhea, herpes, and HPV.
- Giving oral sex carries very low HIV risk but a moderate risk of throat infections such as gonorrhea and herpes.
While oral sex is generally considered lower risk—especially for HIV—it still presents a meaningful risk for several other STIs.
Risk can be reduced by:
- Avoiding contact when sores, cuts, or gum bleeding are present
- Avoiding ejaculation in the mouth if concerned about HIV
- Using condoms or dental dams
- Getting regular STI testing, including throat swabs where relevant
And the most safe - avoid punting completely. This is my favorite one
