I don't think it's particularly bad, just unsightly and I imagine awkward - "Hey Guy, what are those pustules on yer face...?"
Diseases tend to spread exponentially at first, which means if only a few cases are visible there could be twice as many unseen cases. So by the time people see it as a "problem" it's much harder to manage. A lot of people don't understand that.

I'm pretty sure it's spread by touching the affected area.

I did see a adult film actor talk about getting Mpox and it's progress, was a while ago now.
