Back in 19th century the procedure called a lithotomy was used to remove bladder stones. This was done without anaesthetic and required a long metal rod being inserted into the penis and then being banged on with a small hammer until the stones had disintegrated enough for them to pee out. This was done up until the mid 1800's and was an extremely painful process.
